Open Enrollment ImageOpen Enrollment for Medical, Dental and Vision Benefits begins Monday November 6, 2017 and ends November 19, 2017 at 11:59 p.m.

All employees eligible for coverage must enroll or decline coverage online for medical,dental and vision benefits.  Coverage is effective January 1, 2018 through December 31, 2018.  The consequence for failure to complete the open enrollment is no coverage for calendar year 2018.  We also use the waivers as our documentation for the $500 bonus so you need to decline coverage online.

If you want to keep your same plan you must enroll online. There is no automatic rollover.

New employees, yes you must reenroll because this is for calendar year 2018.

If you have a spouse that is covered as a dependent you will be prompted to provide a redacted version of your 2016 Federal Income Tax Form 1040. This has become an annual process so you may want to start keeping a redacted copy where all the dollars amounts and the first 5 digits of your SSN are blacked out. You will need to scan that document and upload it, this can be as simple as a picture of it from your smart phone.

Attached you will find several documents that will provide information and instructions on the open enrollment process. Premiums will go up by 6% January 1, 2018 and our rates remain very competitive.
